COURSE SUBSTITUTION
Course Substitution Form - Student completes form and sends a copy of the the completed form to the Department Chair. The Department Chair answers the questions posed and sends by email to the Registrar’s Office

INCOMPLETE GRADE ASSIGNMENT FORM and Directions
This form is used to assign an incomplete grade - Extension - this form is only for use when an incomplete grade was previously assigned and an extension is necessary: Extension of Deadline for Previously Assigned Incomplete Grade

GRADUATION
INDEPENDENT/DIRECTED STUDIES AND INTERNSHIPS/PRACTICA
PLACEMENT EXAMS FOR WLL OR MUSIC - RECORDING PLACEMENT FOR STUDENTS
- Record an exam placement for a language that does not have an online exam (Arabic, Greek, Hebrew, Latin, Russian)
