Self-Assessments
Tools to help individuals assess their substance-related risk.
Purpose
These self-assessments are offered with the intention of empowering individuals to gauge their risk of experiencing substance-related difficulties. If you have questions about them or experience technical difficulties, please reach out to our office.
Assessments
Understanding your relationship to a substance isn’t as simple as a single score. If you’re interested in another way to reflect on the role a substance plays in your life, try one of the resources below.

If there is another type of assessment you’d like to see available, please reach out to healthed@lclark.edu.
Limitations
These screening tools are not intended to serve as a substitute for a complete substance use evaluation. If you feel that it would be beneficial for you to receive substance-related support, please speak with a healthcare provider.
