TEP Funded Equipment and Materials - Inventory & Renewal Form

Items and equipment purchased with grant funds belong to Lewis & Clark. All institutional policies regarding capital equipment, purchasing, and procurement apply.  This includes items such as iPads, tablets, and accessories that are purchased as part of a Classroom Innovation Grant.  As such, the Teaching Excellence Program performs a physical inventory of equipment at the end of each academic year to verify existence, condition, and accuracy of records.  At the time of inventory, the awardee may request to continue use of the item by completing the following renewal form.

Individuals who leave the College should arrange to return items before their last day on campus.
